About Redux Rehab and this agreement
Redux Rehab Pty Ltd (ABN 36 696 717 432) is a workplace rehabilitation provider approved by the State Insurance Regulatory Authority (SIRA) for the NSW workers compensation scheme (Same Employer stream). We also provide rehabilitation services under the NSW CTP (motor accident) scheme.
SIRA is the New South Wales government regulator for the workers compensation and CTP schemes. It sets the standards providers must meet and approves the workplace rehabilitation providers permitted to work in the workers compensation scheme. That approval is not automatic. It requires evidence of appropriately qualified staff, documented service and privacy systems, secure record keeping, complaint handling and ongoing quality review, and it is granted for a fixed period and renewed only on reassessment. Redux Rehab holds a current SIRA approval, and SIRA publishes its list of approved providers so that you can confirm this independently.
The NSW workers compensation scheme supports people injured at work. The NSW CTP scheme supports people injured in a motor accident. Both are built on the same principle: that a safe, well planned return to your normal life and work is part of your recovery, not something that waits until your recovery is finished. Services provided under either scheme are funded by the insurer, not by you.
It helps to know who is who. SIRA sets the rules and approves the providers. Behind most workers compensation claims in New South Wales is icare, the state government insurer, and your claim is managed day to day by one of its scheme agents, such as EML, Allianz, Gallagher Bassett or QBE, which is where your case manager sits. Some large employers are licensed to insure their own workers and manage claims themselves, and CTP claims are handled directly by the motor accident insurer. Your employer provides the suitable duties you return to. Your nominated treating doctor decides what you are medically fit for and records it on your certificate of capacity. Nobody in that chain can overrule your doctor on your capacity.
A workplace rehabilitation provider, also called an occupational rehabilitation provider, is the service that connects those parts and plans and coordinates that return. We are independent of your treatment. We do not treat your injury, we do not replace your doctor, your physiotherapist or any other practitioner you see, and your choice of treating team remains entirely yours. Our work is to understand both your injury and your job, establish what you can safely do now, and bring your treating doctor, your employer and your insurer to a plan that everyone has agreed on. In practice that can include assessing your duties or your workplace, developing a recovery at work plan and suitable duties, arranging any adjustments or equipment you need, and staying with the plan as your capacity changes.
You are completing this form because you have spoken with Redux Rehab or your referrer and verbally agreed to Redux Rehab acting as your rehabilitation provider. Signing below makes that agreement formal. Under both schemes your insurer approves the referral and funds the program, so once you sign we send your request and your consent to your insurer, and your program starts when they approve it.
Your right to choose your provider
You have the right to choose your own rehabilitation provider. For a NSW workers compensation claim you may choose any SIRA approved workplace rehabilitation provider, and Redux Rehab is one available option. There is no obligation to proceed with us, and your decision will not affect any service, entitlement or relationship with your referrer, employer or insurer.
You can change provider at any time by telling your insurer or case manager.
Conflict of interest disclosure
Redux Rehab Pty Ltd is jointly owned by its two directors: Andrew Ellis, who also owns PACT Physio & Rehab (PACT Health Pty Ltd), and David Parkinson, who also owns Vital Axis Health. These are separate businesses offering different services. PACT and Vital Axis provide treatment. Redux Rehab plans and coordinates your recovery at work.
A conflict of interest can be actual, potential or perceived. An actual conflict is where competing interests genuinely influence a decision being made about your claim. A potential conflict is a connection that could become one if circumstances changed. A perceived conflict is a connection that could reasonably look to someone else as though it might influence a decision, even where it does not.
Where your referral comes from PACT Physio & Rehab or Vital Axis Health, we record that ownership connection as a potential and perceived conflict of interest, and we disclose it to you so that your decision to proceed is a fully informed one.
This structure is neither hidden nor irregular. The directors' interests in these businesses were declared to SIRA in Redux Rehab's application for approval, together with our conflict of interest policy, our conflict of interest register and the management plan that governs referrals of this kind. SIRA assessed that application with those interests fully disclosed and granted Redux Rehab its approval as a workplace rehabilitation provider. Holding an interest in more than one health business is lawful and not uncommon in this sector. What the scheme requires, and what we do, is that such interests are declared openly, recorded, actively managed, and never permitted to influence the professional decisions made about your recovery.
Your treating practitioners and your rehabilitation consultant each work to their own professional judgement. What is recommended about your recovery is a clinical decision made by the person in front of you, not a management one, and no director instructs a practitioner on how to manage your case. The same person does not act as both your treating practitioner and your rehabilitation consultant on the same claim. The businesses are separate legal entities with separate finances and systems, no referral fees, commissions or incentives of any kind pass between them, and the director connected to the referring practice takes no part in the management of your case.
In line with the conflict of interest policy in Redux Rehab's SIRA approval, connections of this kind are disclosed to you, your employer and your insurer at the time of referral and recorded in Redux Rehab's conflict of interest register. You remain free to choose any other provider, now or at any time.
Your information and your privacy
To deliver your program we collect personal and health information relevant to your rehabilitation: your contact details, employment details, claim details, and information about your injury, treatment and capacity.
With your consent, we share relevant information with the people supporting your claim: your insurer or scheme agent, your treating doctor and treating practitioners, your employer (limited to what supports your recovery at work, such as certified capacity and suitable duties, never your broader clinical information), and SIRA where the law requires.
We handle your information under the Privacy Act 1988 (Cth), the Health Records and Information Privacy Act 2002 (NSW) and our Privacy Policy at reduxrehab.com.au/privacy-policy. Your records are stored securely in Australia and retained for at least 7 years from your last service, as the law requires.

Participation, costs and stopping
Your participation is voluntary. You can withdraw your consent at any time by telling us in writing. If you withdraw, we stop providing services and advise your insurer, as the scheme requires.
Rehabilitation services under workers compensation and CTP are billed to the insurer. There is no cost to you.
This form and your signature are collected electronically. A signed PDF copy is stored on your confidential patient record, and you can ask us for a copy at any time.
